How the tiers work
Most peptide tier lists rank compounds by reputation, anecdotes or how popular they are. This one ranks only the evidence: how much published research in people exists, and whether a regulator has reviewed it. A compound moves up a tier when that evidence changes, not when it becomes more popular.
- Tier S, fda-approved. Approved by the FDA for at least one specific indication.
- Tier A, strong human trials. Randomized human trials published in major journals, or a former FDA approval, but not currently approved.
- Tier B, some human evidence. Human studies or approvals outside the US exist, but the evidence is smaller, older, regional or mixed.
- Tier C, early human data. Pharmacokinetic, pilot or safety studies in people; the effects people talk about come mostly from animal research.
- Tier D, mostly preclinical. The evidence comes almost entirely from animal and laboratory studies.
A higher tier does not mean a compound is right for anyone, and a lower tier does not mean it has been shown not to work. Approval in tier S covers one specific medical use, under a prescriber's care.

What a tier does not tell you
Approval is for a product and a use. Semaglutide's approval covers Wegovy and Ozempic for their labeled indications. A compounded or research-grade vial of the same compound is a separate product that no regulator has reviewed.
Evidence is not the same as safety for you. Even tier S medicines have known side effects and reasons not to use them, which is why they are prescribed. Each compound page lists the side effects reported in research.
Research-chemical quality is unverified. Outside approved products, nobody independently checks what is in a vial or how pure it is.

Frequently asked questions
What is the best peptide?
There is no single best peptide, because each one is studied for different purposes. This list does not rank how well peptides work. It ranks how much published human evidence exists for each one, from FDA-approved medicines in tier S to compounds studied mostly in animals in tier D.
Which peptides on this list are FDA-approved?
12 are in tier S: Semaglutide, Tirzepatide, Tesamorelin, PT-141 (bremelanotide), Liraglutide, Orforglipron, Somatropin (HGH), Pentosan Polysulfate, Teduglutide (GLP-2), Oxytocin, SS-31 (Elamipretide), Melanotan I (Afamelanotide). Each is approved only for a specific indication, and use outside that indication is off-label.
Why are BPC-157 and TB-500 in tier D?
Most of the research on both comes from animal and laboratory studies, and human data is limited. Both are also on the World Anti-Doping Agency's prohibited list. Tier D describes the evidence, not a verdict that a compound does nothing.
If a peptide is in tier S, is a vial I buy online FDA-approved?
No. The approval covers the branded medicine, such as Wegovy or Zepbound. Compounded or research-grade powder vials are separate products and are not themselves FDA-approved, even when the compound is the same.
How often is this list updated?
When new trial results or regulatory decisions change a compound's evidence. The date at the top of the page shows the last review.
Which peptides are mostly preclinical?
Tier D: BPC-157, TB-500, KPV, Epithalon, 5-Amino-1MQ, IGF-1 LR3, IGF-1 DES (1-3), PEG-MGF, Follistatin-344, HGH Fragment 176-191, Adipotide (FTPP), Dihexa, P21 (P021), Pinealon, FOXO4-DRI, Humanin, Livagen. Their evidence comes almost entirely from animal and laboratory studies.
