What it is
Petrelintide is a long-acting amylin analog developed for obesity. Amylin is a hormone co-secreted with insulin from the pancreas that slows gastric emptying and promotes meal termination through central (brain-based) mechanisms. Amylin analogs promote satiety, the feeling of fullness, through a mechanism distinct from GLP-1 and other incretin-based treatments.
In the PeptidesAI encyclopedia, petrelintide is categorized under weight management. It remains investigational: the human data described on this page come from phase 1 and phase 2 trials.
How it's thought to work
Petrelintide mimics amylin, the pancreatic hormone co-secreted with insulin, which slows gastric emptying and promotes satiety. The authors of the phase 2 trial describe this as a mechanism of action distinct from incretin-based treatments.
What the research says
In ZUPREME-1, a phase 2 trial in 493 randomized adults with obesity and without type 2 diabetes, the mean change in body weight after 28 weeks ranged from -7.9% to -9.8% across the five petrelintide groups, compared with -1.7% on placebo, and body weight continued to decline to week 42, with reductions of up to -10.7%.
In two phase 1 trials, petrelintide had a half-life of about 10 days and reduced body weight by up to 8.6% after 16 weeks.
Reported side effects
In the phase 2 trial, the most common adverse event with petrelintide was nausea (20% of participants versus 6% on placebo), predominantly mild and occurring during dose escalation. Vomiting (3% versus 6%), diarrhea (7% versus 7%) and constipation (7% versus 4%) were infrequent. In the phase 1 trials, most gastrointestinal adverse events were mild and none was serious or severe.
Reconstitution
Petrelintide is supplied as a lyophilized (freeze-dried) powder and is typically reconstituted with bacteriostatic (BAC) water before use. It commonly comes in 5 mg and 10 mg vials. The amount of peptide in the vial is fixed; the water added only changes how concentrated the solution is.
Worked example. A 5 mg vial is 5,000 mcg. Reconstituted with 1 mL of BAC water, that works out to 5,000 mcg ÷ 1 mL = 5,000 mcg/mL. On a U-100 insulin syringe, where each unit is 0.01 mL, each unit holds 5,000 × 0.01 = 50 mcg. Use the peptide reconstitution calculator to work out the concentration and units for any vial size and water volume.
Important notes
Petrelintide is an investigational compound and is not FDA-approved. Material sold as a research chemical is not a regulated medicine, so its quality and purity are not independently verified.

Frequently asked questions
These questions cover the same ground as the sections above, in a shorter format.
What is petrelintide?
A long-acting amylin analog in clinical development for obesity. It is investigational and not FDA-approved.
How is petrelintide thought to work?
It mimics amylin, a hormone co-secreted with insulin that slows gastric emptying and promotes satiety, through a mechanism distinct from GLP-1.
What does the research on petrelintide say?
A phase 2 trial in 493 adults with obesity reported mean body weight changes of -7.9% to -9.8% at 28 weeks across petrelintide groups versus -1.7% on placebo.
What are the reported side effects of petrelintide?
Mainly nausea, predominantly mild and during dose escalation (20% versus 6% on placebo); vomiting, diarrhea and constipation were infrequent.
How much is one unit from a 5 mg Petrelintide vial?
With 1 mL of BAC water, a 5 mg vial is 5,000 mcg/mL, so one unit on a U-100 syringe (0.01 mL) holds 50 mcg.
